Primary care groups and health insurers will start pilots with patient profiles to assess the need for care in patient populations.
This could be a future means of contracting care for chronic diseases. The uses of patient profiles facilitates the working with individual patient care plans, one of the main desires of patient advocacy groups.
This has been the outcome of a meeting held March 29th at ZN, where 60 representatives of Dutch health insurers and primary care groups discussed the ‘model chronic care program’ which is being developed by ACSION and Casemix.
